TMJ & Jaw Pain
Why Your Jaw Clicks, Aches, or Locks — and What the Neck Has to Do With It
Your temporomandibular joints (TMJ) — one just in front of each ear — are among the most used joints in your body, cycling thousands of times a day through talking, chewing, and swallowing. TMJ dysfunction shows up as jaw clicking or popping, aching in the jaw or face, difficulty or pain with opening wide, morning jaw fatigue from night clenching, ear fullness, and headaches at the temples. Here is what most people with jaw pain are never told: the jaw and the neck function as one mechanical system. Head posture directly changes how the jaw tracks, the upper neck and jaw share nerve pathways, and research and clinical experience alike show that neck dysfunction and TMJ dysfunction routinely travel together. Treating one while ignoring the other is why so much jaw pain lingers.
What causes TMJ dysfunction
The common drivers: clenching and grinding — often stress-related, often during sleep — which overloads the joint and fatigues the chewing muscles; forward-head posture, which pulls the lower jaw backward in its socket and changes joint tracking with every bite (the same tech-neck pattern behind so much modern neck pain); upper cervical joint restriction, which alters the resting position and muscle tone of the entire jaw system; trauma, from a blow, a fall, or even a long dental appointment; bite (occlusion) issues and dental factors; and displacement of the joint's small cartilage disc, the usual source of clicking. Most cases involve several of these at once, which is why single-fix approaches disappoint.
How we evaluate it at Vitality Chiropractic
We evaluate the jaw and neck together, because that's how they work. Your examination includes TMJ assessment — opening range and pattern (does the jaw deviate?), joint sounds, palpation of the joint and the chewing muscles inside and outside the jaw — plus a complete cervical spine examination, with particular attention to the upper neck segments that share nerve supply with the jaw, and postural assessment of head carriage. In our clinical model, restricted upper cervical joint motion affecting muscle tone and nerve function — vertebral subluxation — is a frequent and correctable contributor to jaw dysfunction. Thermal scanning and HRV testing provide objective baselines, and cervical X-rays are taken when clinically indicated.

What care looks like
Care addresses the whole system: gentle, specific work to the TMJ itself to improve joint tracking; precise upper cervical adjusting to restore the neck mechanics the jaw depends on; soft-tissue therapy for the chewing muscles, including the ones inside the mouth that almost never get treated; Avant laser therapy over the joint to reduce inflammation and pain; and postural correction — because a jaw can't track normally on top of a forward-carried head. You'll also get practical home strategies: jaw rest positioning, habit awareness for daytime clenching, and stress-management coaching from our six home basics, since stress is fuel on this particular fire. As always, your plan is written with scheduled re-evaluations.
When chiropractic first makes sense — and when we refer
Muscle- and joint-driven TMJ dysfunction — the majority of cases — is well suited to conservative care first. We work alongside dentists routinely and refer when the picture calls for it: significant bite problems, suspected disc displacement with a jaw that locks closed or open, night grinding that warrants a professionally fitted appliance, or dental pathology masquerading as joint pain. Ear pain with fever or hearing changes goes to a physician to rule out ear disease. Jaw pain with chest pressure, shortness of breath, or arm pain — especially during exertion — is treated as a cardiac emergency, not an appointment. Our examination screens for all of it before care begins.
Frequently asked questions
Yes — TMJ dysfunction is a musculoskeletal condition, squarely in our lane. The jaw is a joint moved by muscles and supplied by nerves it shares with the upper neck, and our care addresses the joint, the muscles, and the neck mechanics together.
Clicking usually means the joint's small cartilage disc is slipping rather than gliding smoothly. Painless clicking is common and not an emergency; clicking with pain, or a jaw that catches or locks, deserves an evaluation.
If night grinding is a significant driver, a professionally fitted appliance from your dentist protects your teeth and joint — and we'll tell you if your examination points that way. A guard manages the load; our care addresses the mechanics underneath it. They work well together.
Routinely. The upper neck and jaw share nerve pathways, and forward-head posture mechanically changes jaw tracking — which is why neck care is part of nearly every TMJ plan in our office, and why temple headaches often improve alongside.
