Pregnancy-Related Back Pain
Why Your Back and Pelvis Hurt During Pregnancy — and the Gentle Care That Helps
Most pregnant women experience back or pelvic pain at some point — low back ache, sharp sacroiliac pain on one side, pubic bone pain, or sciatica-like pain into the leg. It is so common that many moms are told to simply endure it until delivery. We disagree, and so does the evidence: pregnancy-related back and pelvic pain is musculoskeletal, it has identifiable mechanical causes, and gentle, pregnancy-specific chiropractic care is a safe, drug-free approach that guidelines and research support for musculoskeletal pain in pregnancy. Every doctor at Vitality Chiropractic is Webster Certified through the ICPA — the recognized certification for chiropractic care of pregnant patients — and expecting moms are one of the largest groups we serve.
What causes back and pelvic pain in pregnancy
Pregnancy remodels your mechanics from several directions at once. The hormone relaxin loosens the ligaments of the pelvis to prepare for birth — necessary, but it means the sacroiliac joints and pubic symphysis lose stability and can become irritated and restricted. Your center of gravity shifts forward as the baby grows, increasing the curve of the low back and loading the facet joints and muscles that support it. Abdominal muscles stretch and provide less support. And postural compensations stack up week by week. The result: SI joint pain (often one-sided, near the dimples of the low back), pubic symphysis pain (front and center, worse with stairs and rolling in bed), general low back ache, round ligament pain, and true pregnancy sciatica when pelvic changes irritate the sciatic nerve.
How we evaluate it at Vitality Chiropractic
Pregnant patients receive a complete, pregnancy-appropriate evaluation: detailed history, physical examination, motion testing of the spine, sacroiliac joints, and pelvis, and assessment of the muscles and ligaments that stabilize them. We never take X-rays during pregnancy — no exceptions. Thermal scanning and HRV testing, which are entirely safe in pregnancy, give us objective nervous system baselines to measure progress against. In our clinical model, we're identifying the restricted, dysfunctional joints — vertebral subluxation — and the pelvic imbalance pattern behind your specific pain, because 'pregnancy back pain' is not one condition, and the care plan depends on which pattern you have.

What care looks like
Every adjustment is adapted for pregnancy: specialized pillows that let you lie face-down safely and comfortably (many moms tell us it's the best they've felt in months), gentle low-force techniques, and positioning that respects your changing body at every stage. The Webster Technique — a specific, gentle analysis and adjustment of the pelvis — is a cornerstone of our pregnancy care: by restoring balance to the pelvic joints, muscles, and ligaments, it addresses the mechanical root of most pregnancy-related pelvic and low back pain. Around the adjusting: soft-tissue care for the round ligaments and hip stabilizers, safe home exercises and positional strategies for sleep and daily activity, and coaching from our six home basics — hydration, movement, and sleep matter double right now. Care frequency typically adjusts through the trimesters, and many moms continue supportive care right up to delivery.
When chiropractic first makes sense — and when we refer
For musculoskeletal pain in pregnancy — back, pelvic, hip, sciatic — gentle conservative care is a first-line option precisely because the usual alternatives (medications, imaging, injections) are limited during pregnancy. We stay firmly in the musculoskeletal lane, and we co-manage: we routinely communicate with OBs and midwives, and we refer immediately for anything outside our scope — bleeding, leaking fluid, severe headache with visual changes, significant swelling, decreased fetal movement, fever, or pain that doesn't behave mechanically. Those go to your provider or straight to labor and delivery, and we'll be the first to say so.
Frequently asked questions
Yes — with a properly trained provider using pregnancy-adapted techniques, chiropractic care is widely recognized as safe throughout pregnancy. Every doctor on our team is Webster Certified through the ICPA, and we care for expecting moms every single day, in every trimester.
A specific, gentle chiropractic analysis and adjustment of the pelvis, developed for pregnancy, that restores balance to the pelvic joints, muscles, and ligaments. It's the recognized standard for prenatal chiropractic care, certified through the ICPA — and every one of our doctors holds it.
Whenever you'd like — many moms start in the first trimester before pain begins, others come in the third trimester when the pelvis is under the most strain. Earlier tends to be easier, because we're preventing patterns instead of unwinding them.
Never. Your evaluation uses examination, motion testing, and radiation-free tools like thermal scanning and HRV testing only.
The honest answer: the Webster Technique addresses pelvic balance — it is not a technique for turning babies, and we won't claim it is. A balanced pelvis supports optimal space and function, and we work alongside your birth provider, who manages everything related to your baby's position and delivery.
