Why Does My Heel Hurt With the First Steps of the Morning — and What Actually Fixes It?

Plantar fasciitis is an overload injury of the plantar fascia — the thick band of connective tissue that runs from your heel bone to the base of your toes and supports your arch with every step. The classic sign is sharp heel pain with the first steps of the morning or after sitting, easing as you move, then returning after long days on your feet. Because the plantar fascia is soft tissue with limited blood supply, it heals slowly on its own — which is why the most effective care combines treatment that stimulates tissue healing directly, like SoftWave acoustic wave therapy, with correction of the mechanical problems up the chain that overloaded the fascia in the first place.

What causes plantar fasciitis

The plantar fascia acts like a bowstring under your arch. Every step loads it; healthy tissue tolerates that load and repairs overnight. Plantar fasciitis develops when the load chronically exceeds the tissue's capacity to recover — from a rapid increase in walking, running, or standing; unsupportive footwear or hard surfaces; tight calf muscles and a stiff ankle, which force the fascia to absorb motion the ankle should provide; excess body weight; and foot mechanics like flat arches or high rigid arches that concentrate stress at the heel attachment.

What often goes unexamined is everything above the foot. The foot is the end of a kinetic chain that starts at your pelvis and low back. A pelvis that has lost normal motion, a restricted hip, or an altered gait pattern — sometimes from an old injury you've long forgotten — changes how force travels into the foot with every stride. That is frequently the reason one foot develops plantar fasciitis and the other doesn't, and it is exactly the kind of finding a foot-only approach misses.

How we evaluate it at Vitality Chiropractic

Our clinical model looks for the mechanical and neurological dysfunction behind a symptom, not just the spot that hurts. For heel pain, that means we examine the foot and ankle directly — palpating the fascia attachment, checking ankle and big-toe joint motion, and assessing calf flexibility — and then we evaluate the entire chain: gait, knee tracking, hip motion, pelvic alignment, and the lumbar spine and sacroiliac joints. Restricted spinal and pelvic joint motion — what our model calls vertebral subluxation — alters movement patterns downstream, and our examination is built to find it. Where clinically indicated, spinal thermal scanning and heart rate variability (HRV) testing give us an objective baseline of nervous system function, and digital X-rays show us pelvic and spinal structure so care is built on findings, not guesses.

When chiropractic first makes sense — and when we refer

Most heel pain is mechanical, and conservative, non-drug care is the appropriate first step — guidelines consistently recommend exhausting conservative treatment before injections or surgery are considered. We refer when the examination suggests something other than plantar fasciitis: signs of a calcaneal stress fracture, a sudden 'pop' with acute arch collapse suggesting fascia rupture, nerve entrapment requiring further workup, or inflammatory arthritis. And if your findings aren't improving on schedule at re-evaluation, we will tell you and coordinate with a podiatrist or your physician rather than string care along. That honesty is part of how we practice.

Frequently asked questions

How long does plantar fasciitis take to heal?

Chronic cases that have lingered for months rarely resolve in a week or two — tissue remodeling takes time. Most patients notice meaningful improvement within the first several weeks of combined SoftWave and mechanical care, with re-evaluations built into the plan so you can see objective progress rather than guessing.

Do I need orthotics?

Sometimes — but not always. Supportive footwear matters for everyone; permanent custom orthotics are a case-by-case decision we make after your evaluation, not a default upsell.
