What a Herniated Disc Actually Is — and Why Surgery Is Rarely the First Answer

A herniated disc happens when the soft inner core of a spinal disc pushes through a weakened area of its tough outer wall, where it can press on or chemically irritate a nearby nerve root — producing back or neck pain, and often pain, numbness, or weakness radiating down an arm or leg. Here is the most important thing to know: research consistently shows that most disc herniations improve with conservative care and time, and that herniated material commonly shrinks on its own over months. Clinical guidelines recommend non-drug, non-surgical care as the first step for most cases — and our office is specifically equipped for disc care, including Cox Flexion-Distraction technique and lumbar decompression traction.

What causes a disc to herniate

Discs rarely fail from one bad lift alone. They fail from years of accumulated stress — prolonged sitting and flexion that load the disc's rear wall, restricted motion at spinal segments that concentrates force instead of distributing it, poor hinge mechanics with bending and lifting, and the normal drying of disc tissue with age — until an ordinary movement becomes the final straw. That's why 'I just bent over to pick up a sock' is such a common story. The herniation is the event; the mechanics that produced it built up quietly for years, and addressing those mechanics is what protects the disc long-term.

How we evaluate it at Vitality Chiropractic

Disc cases demand a careful examination before anyone is adjusted. Your evaluation includes a detailed history, orthopedic and neurological testing — nerve tension tests, reflexes, muscle strength, and sensation mapping to identify which nerve root is involved and how much it's compromised — and joint-by-joint assessment of the spine and pelvis, because the segments above and below a herniation are usually part of the mechanical story. In our clinical model, we're looking for the pattern of restricted segmental motion and nerve involvement — vertebral subluxation — that explains both the pain and the path back out of it. Thermal scanning maps dysfunction along the spine, HRV testing gives a nervous system baseline, and digital X-rays, when clinically indicated, show alignment, disc spacing, and degenerative change. We also screen every disc case for red flags before care begins.

Around the mechanical work: SoftWave therapy and Avant laser to address inflammation and support tissue healing, careful adjusting of restricted neighboring segments, and — once the acute phase settles — progressive rehabilitation to rebuild the hinge and lifting patterns that protect the level for good. Every plan is written, with milestones and scheduled re-evaluations.

When chiropractic first makes sense — and when we refer

Guidelines are clear that most disc presentations don't need an MRI before starting conservative care — imaging is reserved for cases where the result would change the plan. We refer for advanced imaging, and when indicated to a spine specialist, when the examination shows progressive muscle weakness, spreading numbness, symptoms worsening despite an appropriate course of care, or red flags in the history. One presentation is an emergency, not an appointment: saddle-area numbness or new changes in bladder or bowel control require immediate emergency evaluation. Our examination screens for all of these before care begins, and our re-evaluation schedule means a case that isn't responding gets escalated promptly.

Frequently asked questions

Can an adjustment push the disc back in?
No — that's not how discs work. Care restores motion and reduces mechanical irritation so your body's own healing and resorption processes can do their job; herniated material commonly shrinks over months.
Do I need an MRI before you'll see me?
Usually not. Your history and examination determine whether imaging is needed first, and we'll tell you honestly if it is.
Is flexion-distraction painful?
It's a gentle, non-thrusting technique most disc patients find relieving — many describe it as the most comfortable their back feels all day.
How do I know if my leg pain is from a disc?
Radiating pain below the knee, especially with numbness or weakness in a specific pattern, points toward nerve root involvement — but sciatic-type pain has several possible causes, which is exactly what the examination sorts out.
