Carpal Tunnel and Wrist Pain: Is the Problem Really in Your Wrist?
Carpal tunnel syndrome is compression of the median nerve as it passes through a narrow tunnel at the front of your wrist, producing numbness, tingling, and weakness in the thumb, index, and middle fingers — often worst at night. But here is what many people are never told: the median nerve starts in your neck, and it can be irritated anywhere along its path — at the cervical spine, under tight chest and shoulder muscles, at the elbow, or at the wrist itself. When the nerve is compromised at more than one point (clinicians call this 'double crush'), treating only the wrist often fails. That is why our evaluation traces the entire nerve path before anyone treats anything.
What causes carpal tunnel and wrist pain
True carpal tunnel syndrome usually develops from sustained wrist positions and repetitive hand use — keyboard and mouse work, assembly work, tools that vibrate, gripping — which thicken the tendons sharing the tunnel and squeeze the nerve. Fluid changes in pregnancy commonly trigger temporary carpal tunnel symptoms as well. Thyroid conditions and diabetes raise risk, which is part of a thorough history.
Just as often, hand numbness and wrist pain trace partly or entirely to somewhere else: nerve root irritation at the neck (which can also produce arm pain and altered reflexes), restricted upper-back and rib mechanics that tighten the muscles the nerve passes beneath, or wrist joint restrictions from an old fall on an outstretched hand. Forward-head desk posture — the same pattern behind tech neck — is a frequent background contributor, loading the exact structures the median nerve threads through.

We never assume the wrist is the whole story. Your examination includes orthopedic and neurological testing along the full nerve path — cervical spine testing, provocative tests at the shoulder, elbow, and wrist, plus strength, sensation, and reflex checks — alongside joint-by-joint assessment of the neck, upper back, elbow, and the small bones of the wrist for restricted motion. In our clinical model, restricted spinal joint motion affecting nerve function — vertebral subluxation — is a primary suspect whenever nerve symptoms appear in a limb, and it's why the neck is examined even when the hand is what tingles. Thermal scanning and HRV testing establish an objective nervous system baseline, and cervical X-rays are taken when clinically indicated.

When chiropractic first makes sense — and when we refer
Mild to moderate symptoms — intermittent numbness, night waking, achiness with use — are appropriate for conservative care first, and that's the sequence most guidelines support before surgery is considered. We refer without hesitation when the examination shows signs of advanced nerve compromise: constant numbness that never fully clears, visible wasting of the muscle pad at the base of the thumb, progressive weakness, or dropping objects. Those cases need nerve conduction testing (EMG/NCV) and a surgical consultation, and delaying that referral risks permanent nerve damage. If that's what we find, we'll tell you at the report of findings — not after months of care.
What care looks like
Care is matched to where your nerve is actually being irritated. That typically combines specific adjustments to restore motion to restricted cervical, upper-back, elbow, and wrist joints; soft-tissue work for the forearm flexors and the muscles the nerve passes beneath; Avant laser therapy at the wrist to reduce inflammation around the nerve; SoftWave therapy for chronic tendon thickening when the case calls for it; nerve-gliding exercises for your home program; and ergonomic correction of the workstation or task that created the overload — because care that ignores eight daily hours of provocation doesn't hold. Night splinting may be recommended in the early phase. Your plan includes re-evaluations where we re-test strength and sensation against your baseline.
Frequently asked questions
You usually can't tell from symptoms alone — that's what the examination is for. Neck-driven symptoms often involve the whole arm or change with head position; true carpal tunnel is confined to the thumb-side fingers. Many people have contributions from both, which is exactly why we test the entire nerve path.
For mild to moderate cases driven by mechanical compression and repetitive strain, conservative care frequently resolves symptoms. Severe cases with constant numbness or muscle wasting typically need surgical decompression, and we will refer you honestly if your examination shows that.
Very commonly, yes — fluid shifts in pregnancy narrow the tunnel. It's usually temporary, and gentle conservative care plus night splinting helps most moms manage until it resolves after delivery. Our entire team is Webster Certified and cares for pregnant patients daily.
Only when clinically indicated — typically of the neck when the examination suggests cervical involvement. We don't X-ray by routine, and never during pregnancy.
